School Forms:
  • Field Trip Permission Slip (Overnight Trip)
  • Field Trip Permission Slip (Day Trip)
  • Athletic Department Forms - Forms must be returned to the coach and nurse before any participation in Sports at BHS- The Blue CT Health Assessment form below, can NOT be substituted for the Athletic Department Physical form.  The Completed Sports Physical form must be returned to the Nurse's office before participation in any sport.  An athlete must submit the completed Training Rules and Permission Slip forms to his coach for EACH sport he or she participates.
  • CT State Health Assessment Form - must be completed by Junior Year and returned to the Health Room.
